Information Change FORM

Please do not use this form if you are not the parent/guardian of an actively enrolled child.

Authorized pickup person(s) cannot be added/removed via this form. If you, a parent/guardian, wishes to add a person to the list of individuals authorized to pick your child up, you must submit a written authorization letter to the main office, in-person with your signature.

This form is primarily used to update address, phone number, and email information for parents of currently enrolled children.
