Please do not use this form if you are not the parent/guardian of an actively enrolled child.

A 30-day advanced notice is required for each child or children whose schedule you plan to change, for any reason.
Insufficient notice therefore does not guarantee the prospective schedule slot’s availability.

This form is primarily used to request a schedule change for your currently enrolled child.
