EXTENDED CARE: 1st to 3rd Grade

Enrollment Information

1. Call Elmhurst to schedule a tour to view the school. (Please have child attend the tour.)

2. If you decide to enroll, pay the Registration & Material Fee to secure your child's placement.

3. You will receive an Enrollment Packet to fill out and return including the following documents:

- Physician’s Report/TB Test

- Proof of Immunization Record

- Birth Certificate

- Parents/Guardians copy of I.D.

4. Sign up on FACTS. *Child will not be able to attend Elmhurst until FACTS is set up.

5. Return the completed packet to the office three days prior to child starting.

Extended Care Program Fees

Registration Fee                                $275.00

Extended Care Material Fee            $150.00

Drop-in Daily Rates

6:45 am to 6:00 pm                            $65.00 / day

6:45 am to 12:45 pm                           $55.00 / day

Extended Care:

1st to 3rd grade Tuition

Full Day                                      $1,150.00 / month

Before & After School              $995.00 / month
