Notice of withdrawal FORM

Please do not use this form if you are not the parent/guardian of an actively enrolled child.

A 30-day advanced notice is required for each child or children leaving Elmhurst Learning Center, for any reason — Whether due to a school transfer, a move, graduation, vacation, ect. We also cannot offer prorated refunds for vacation time.
Insufficient notice will leave parents responsible for a month’s worth of tuition.
There are generally no exceptions to this policy.

This form is primarily used to notify ELC of withdrawal from a current program for a currently enrolled child.
